Changes to the Application for Personal Insurance Submissions

Dear Brokers,

​​​​​​​We have updated our personal lines application to meet the evolving information requirements of our carriers. You can download the new application and the updated checklist from the MPG Private Client Services webpage, PCS Forms and Applications

As a reminder, the following must also be included with your submissions:

• Date of birth for all named insureds.
• Occupation for all named insureds.
• MVR activity for all named insureds. Our carriers no longer pull MVRs pre-binding. Auto and excess quote pricing and acceptability are based on acceptable MVRs.
• Mortgage information. Amount and number of mortgages. The premium and/or the carriers willingness to write the risk may be affected by that information.
• Current insurance carrier. Carriers require this information.
• Flood insurance. If the property is located in a non-preferred flood zone, proof of primary flood insurance and an elevation certificate showing positive elevation depending on the carrier may required.
